Child photo / video consent
We would be grateful if you would fill in this form to give us
permission to take photos of your child and use these in our printed and online publicity.
I grant full rights for Wollaston and Strixton Preschool to use the images resulting from
photography/video filming, and any reproductions or adaptations of the images they
have taken during session time for fundraising, publicity or other purposes to help achieve
the group’s aims. This might include (but is not limited to), the right to use them in their
printed and online publicity, social media, press releases and funding applications.
Tapestry Online Journal
Tapestry is the secure online platform we use to share photographs, observations
and updates about your child’s experiences, learning and development within the
setting. Parents, carers and guardians are able to access their child’s
individual learning journey throughout their time with us. For further information,

Illness
Please advise the Preschool prior to 9.00am if the child will not be attending due
to
illness.
It is a requirement for settings to know why a child is absent to ensure that the child is
safe. Parents agree that a child who is ill (e.g. fever, infection, diarrhoea, communicable
disease, or any other type or illness that may be passed on to others, with the exception of
the common cold) will be kept at home to protect the well-being of staff and other children
in our care. The parents further agree should a child become ill while in our care, that
immediate arrangements will be made to collect the child from the Preschool.
Children will not be allowed to return to the Preschool until they have been symptom free for at
least 48 hours for a fever and 48 hours for sickness and diarrhoea. In some cases, a note from
the
doctor may be necessary. By signing this contract, you are agreeing to staff seeking any
necessary emergency medical advice or treatment during their time at Wollaston and Strixton
Preschool.
Please see our Poorly
Children
policy
for further information.
Withdrawal
Parents agree that a minimum notice of one full month (notice to be given in
writing)
will be given for permanent withdrawal or reducing hours of any child from the setting or agree
to
pay one month's fee in lieu. The responsibility is of the parents to ensure that the notice has
been
one month's fee in lieu. The responsibility is of the parents to ensure that the notice has been
received by the office. No exceptions will be made.
Termination
Wollaston and Strixton Preschool reserves the right to suspend or terminate sessions
of
any child without notice, should it be deemed necessary for the overall safety and well-being of
staff
and/or
other children in our care.
